To expedite your first visit with us, we are providing you an opportunity to download and fill out our medical and dental forms prior to arriving at our office. Many times, we find that our patients do not always have the information needed to complete the forms when they come to the office. By downloading these ahead of time, and completing them at your leisure, you can be assured of the best possible services.

Below there are five forms that you will need to download and complete:

Privacy Consent Form
Office and Financial Polices Form

The Notice of Privacy Practice is an information only download, and does not require any completion. However, you will need to refer to it when completing the Privacy Consent Form. In addition, you may wish to review our office policies and financial responsibility statement and the Notice of Privacy Practice to read and understand.
